[sebhc] Hard-sectored disks

I pinged Herb and he says he will fix the Apple-II reference.

He also says he thinks the Exidy Sourcerer used 16-sector disks,
but like me can't think of any other systems off the top ... they
were not nearly as common as 10-sector disks.
